Ground fault [RE-wrenches]

I have reviewed the photos of the failure and the drawings and have 
determined that the ground fault protection would have prevented damage 
from either theory A or B situations.  

Mounting all power electronics on a fire-proof surface is always a good 
idea IMHO.
