Charge controllers [RE-wrenches]

I know charge controllers will settle into their charge voltage setpoint as
soon as they lock onto the battery bank, but has anyone had problems with
controller failure due to the much higher open circuit PV voltages when they
dis-connect from the battery?  I didn't notice a problem on 12 to 24 volt
systems, but have seen some smoke on a few higher voltage systems.

Since its not uncommon for a charge controller for a 48 volt battery to
"see" 88 to 90 volts from the array when the battery is fully charged, how
much voltage "overhead" do today's charge controllers have built in?
