I also do not want to insult anyone by offering something "Dah" simple,
but could there have been a bent/damaged fuse holder?  I have never used
the Trace product, I've always made my own combiner boxes.  I have seen
things get really hot on a fuse/fuse holder that got banged around in
shipping or something and no longer grabbed the fuse as tightly as it is
designed to.
