The most dramatic example I ever saw resulted in tripping the ground faults
on a 2500 Amp 3 phase service every day @ 3:25 PM.  It turned out to be a
forklift battery charger that had a pretty healthy nick in the AC cable and
every day when a 1st shift driver parked his forklift he ran over the nick
and the concrete would fault out the charger causing the whole plant to go
down.  It was pure luck to have ever found it.  None the less there was
always at least 10 forklifts and multiple electric pallet jacks on fast
charge gassing like mad along with enough extra batteries to run a small
city charging at all times.  The hydrogen sulfide gas was so strong I
couldn't bear to work in this 50'x50' space for more than 10 minutes as a
time.  All this hydrogen gas and a fault in the neighborhood of 500 amps @
480 volts and still no explosion.  Wow
