Yes, you are right -- even a non occupied garage or shed would be required
to have it if it's not solely for the PV system under exception #1.  But I
was thinking of Exception #2: PV equipment and circuits installed on
nonenclosed detached structures including but not limited to parking shade
structures, carports, solar trellises, and similar structures shall not be
required to comply with 690.12.  That seems to pretty clearly cover
ground mounts.

Also we have the exception that conductors originating from arrays not
attached to buildings, that terminate on the exterior of buildings don't
need to be considered as controlled conductors.  If they're going into an
inverter inside a building but the PV array is not on the building, does an
external accessible DC disconnect count as ending those conductors, and the
output from the disconnect is no longer a controlled conductor either?
