UL 1741 Failures [RE-wrenches]

Is anyone aware of any anecdotal or official documentation of any UL 1741
inverter causing any injury by failing to de-energize the inverter AC output
once it was disconnected from the Ac line anywhere in the USA at any time in
the last 5 years?

If so I would very much appreciate your dropping me a line.
